Okay, when you're in, press the escape key, and on that little menu that comes up, there'll be a slider at the left. Drag the dot on that all the way to the top.
Then clikc system, and near the bottom right there'll be the audio device section, if that's correctly set, switch it to something else, then back to the correct one. If not, make sure you set it to the correct recording device.

What I did:
I have the correct version of the build model, I try to change to a newer version of unity. didnt work.
I am fully up to date with the SDK.
Removed the audio, it still give me the error.
Make it a little bit taller, around the same as my other uploaded avatars.
Looking the error up on google, but nothing related to the problem.

Info: It is a model from dec previous year. Created on version 5.6.3p1.
Other characters works fine and well in vr chat.
