I am big on project planning, so I'd plan all the NPCs out. Where they will spawn, what they will do, where they will go, etc. etc.

Figure out which NPCs the player will touch or come into the proximity of. Other NPCs can be in clusters with low level of detail.

Plan all that out, so that you can focus, focus, focus.
