I think there should be actual consequencies to ideology beyond the rather questionable view of fascist and communist regimes getting to justify war goals and get war economy with absolutely no trade-offs. Obviously, I don't think this means PDX intends to defend Hitler or Stalin.
But there is a reason why democracies were on the winning side of WW2 and are the dominant political system today. They have advantages authoritarian states don't - their populations have genuine loyalty, rather than just fear. They are better at coordinating with each other (the Axis often sabotaged each other, while the US and Britain fought as a united front under joint commands). Capitalist economic systems may make mobilization harder, but they also allowed for enormous industrial capacity and higher standards of living.

I think fascist and communist countries should get debuffs to stability, some economic stats (production efficiency in particular) and fascist nations in particular should have a much slower gain in faction points and reduced benefits from faction membership. In exchange, they keep their ability to get war economy in peacetime and should also get a pretty big buff to counter-intelligence since they have the Gestapo/NKVD/whatever bad guy of the week running things

Your grand doctrine selection decides what tactics your armies can use in battle and provides distinct buffs. You also get access to different command sprits.
Mobile warfare improves speed and mobility but requires you to invest a lot in tanks and mechanized, it's less of a bad choice now but it's really only good for nations like Germany or the USSR that get a lot of tank buffs.
Superior firepower buffs artillery and support companies a lot, though IMO this is the weakest doctrine because artillery sucks in Hoi4 unlike in real life where it actually caused about 70 percent of all casualties in WW2.
Grand battleplan generally improves your planning bonuses and logistics. I think it is the strongest doctrine out of the four due to just how much having good supply benefits your troops. It also boosts entrenchment a lot, which makes it the best choice for defensive warfare
Mass assault reduces the combat width of your divisions and allows you to concentrate more firepower at the cost of generally less flat buffs to things. Good for if you have a country with a lot of manpower like China, the USSR, or even the US. I would recommend not using this as the cheesy "human wave" doctrine but rather take the chance to use line artillery and large-width breakthrough divisions to completely crack open enemy lines

If you click the horse twice it should be replaced with 3 trucks, that's how you know it's fully motorized

I feel they're less useful now but they're not bad, basically it screws up the province for you and the AI and damages any troops in the state that gets hit
