most of the emergency doors that are meant to stop water from leaking into the hull were unable to close properly, letting water leak through
emergency equipment was kept in a locked locker the key to which was ususally on the admiral
what else... oh yeah, almost every anti air defence system didn't work
most of the anti air guns were dismantled for spare parts, the 120 mm guns that were supposed to act as backup didn't work due to hydraulics leaks
every missile system except for one didn't work, and that last one relied on a radar system that once used, would disable all communications on the ship, including internal comms
so it was off 90% of the time and when it was on there was 0 coordination between crew members
the engines were also way past their life cycle and barely worked, but that didn't really contribute to how a couple missiles sank it
