you keep recovering some hp

most of the time thats useless

but if you get hit by bunch of stuff, and 4 seconds after you keep degenning hp, then thats a lot more impactful

because in first case you might've been already on full hp so it does nothing

in second case you can just die and you always take the full effect

same applies in middle of combat, most of recoup is gonna be wasted on overhealing, but the negative recoup is never wasted it will always deal damage to you
