I got it out of 'The void" card, which give you random card and by checking people statistics who opening REALLY a lot of divination cards the chances is 1 to 20 000 to do that.

So answer "you either get it droped or no" is pretty real. Pure 50/50.

If you want currency its probably would be easier to invest in some of league mechanique. Like breach (the cheapest one currently).
Then you just slamming breaches tables everywhere with not the worst affixes in towers (that you oppened inbefore), as well as clearing some maps with 'bad layouts' or things like 'hideout waypoints'

Like you can invest in 12 bosses, and the drop rate of some thing is 30%, so you will get 4 of them, but it didn't mean you won't get any at all, because those 30% works over infinite amount of runs.

Well, example: item drop rate 1 per 10 boss runs on average.
You can do 10 runs and don't drop any.

If its 1 per 2 maps: then you just getting it more often in shorter time (as well as this item would less valueble), but when you have 1/10 drop average, it means you need to do REALLY a lot of runs to make this 1/10 chance working.

Like making 100 runs would give you more chances to get to average of 10 dropped items, and doing 1000 runs would be even closer to getting to 1/10 chance.

Probably good way to get at least 150% total rarity (map and character), there is formula somewhere, but 70character+50map is around 150% and get biggest waystone chance rate.
And then just stack quant over it.

higher requirement probably mean higher item level (ilvl) but if the modifiers are exactly the same, it makes no difference. However, higher ilvl means possibly higher modifier tier so if you want to craft (e.g. chaos orb) then the one with higher ilvl would be more interesting

Infernalist, yeah. Flammability mostly. Dogs got Fire Exposure, Splinter and Strip away on him. I try to use Pain Offering but it seem to instantly get blown up whenever I stick it down. I do currently use an Omen Scepter but haven't really messed with Malice.
