It's definitely a user client issue. Perhaps some files in your device is corrupted because of which discord can't reveal media. Try a different discord build or reset your pc with a new installation of windows. But obviously it could also be connection issues. Do you get decent ping in voice calls? Any lag in video calls?

Above all you should notify discord and ask for support with these info
