The way it works is someone reports you, sections of the chat is sent over to microsoft and eventually someone reviews it and they can either decide to send a warning, or temp ban, or ban based on it or just do nothing if its nothing. And they have a criteria on what isn't acceptable. And there is an appeal system. It's for the whole online system part bnut you'd still be fine and dandy for the offline stuff.

well it goes into a whole lot of different areas, for example if you overrepresent something thats already the majority, it wont hurt that faction
if you have lets say a comic or game thats playing in chicago or w/e that spider man game is and every single person is white, you then have a not really accurate picture of that city, and even if you say it's just a fantasy version, you then alienate everyone not fitting into that since they cant related to characters as much etc.
so changing that back to be more aligned to reality is usually not an issue, but taking a black character and portraying it as white is not a smart move for the same reasons just inverted
