Gear 4 is just inflation of the muscles, even abnormally so by his standards. But its probably likely to give him muscle failure when hes older rather than shortening his lifespan

what luffys doing isnt particularly special he's just raising his heartrate so his blood is pumping faster and he was full of open wounds when he was fighting lucci

so the added strain was just putting a toll on him theres no arbitrary amount of lifespan being sapped
