SGN ML is harder than TAG1 but also shorter, so not terrible.
Taras Nabad is a fucking slog, but not as hard as SGN. Some people struggle with some encounters but honestly, there's not many encounters legitimately harder. Just some are waaaay too long.
Similarly Mars Core can be a slog in some encounters. I think Mars Core starts really well, decent mix of hard but not too long, but (assuming you're not using BFG) the fights get way too drawn out after you would normally get the BFG in base game.
World Spear is the opposite, fucking sloooog of a level until you get around to where the fountain fight is in base game, from there the fights are still difficult but overall typically well crafted. There's one fight with a buff totem where you really need to search for the buff totem, and the ending can be rough if you don't use Hammer well."

You can technically "do more" with it, but it's easier, and overcorrects user error in a way that promotes bad gameplay habits. The achievement itself is one that deserves to be cheesed, due to the fact that the BM player base is usually low or filled with toxics. The achievement itself isn’t really one that can be achieved by normal means (for example: a normal achievement is something like finishing a game mode with a certain number of lives, like we all expect), thus people saying to just cheese it (everyone is in agreement on this).

Ultra violence is hard difficulty, Nightmare is extra hard and after that you have Ultranightmare. Same difficulty as Nightmare except one death is game over.

The game's code refers to it as "impossible", and when you try to select it the game says

However, in (2016) and Eternal, it's just a brutally hard difficulty- but one that's still fair and doable.
