Almost everything is maxed out except for brink of death, damage to demon is at 75%, damage to player is 300%, and daze duration is at 75% (nightmare default). I did a campaign run with nearly identical settings except damage to demon was at 50%. Managed to beat the secret encounter in Belly of the Beast.

Clear fodder with your shield. Soldiers and Imps can kill you while you're trying to deal with other stuff if you don't clear them out. Pick a side of an arena and slowly work your way around. Use the shield bash and throw to lockdown pressure units like Revenant, Whiplash etc.
