To copy a comment I made on Reddit, with how many enemies we see on screen in The Dark Ages, a triple mouthed Pain Elemental could very well populate the environment with numerous Lost Souls. Because we’ve seen Lost Souls drain the Shield Saw’s health in one bash, it would open up other flaming skulls to perform kamikaze attacks on the player. This would then make Pain Elementals high priority targets, as just two could swarm the player with their dive bombing minions. It'd be a great inclusion

TDA Arachnotron is just the Mindweaver in terms of design and gameplay, Tyrant is just weaker Cyberdemon which is also in Eternal/TDA, Banshees work the same as Lost Souls in 2016/Eternal/TDA, Vassago gimmicks are just AoE attacks which are less unique in the modern games, Ghouls don't really do anything unique enough to warrant a return, and Shocktroopers don't really have a placement in the enemy roster given how the Troopers attak nowadays
