There is no way someone to accidentally report you, the reporting process takes multiple steps and it is not typically done through email anymore.
A discord moderator can be identified by the badges on his account ( real discord staff will have a hammer crossed with a screwdriver as they're badge)
Real discord staff will never DM you or contact you through DMs for a problem, discord will email you and a Real staff member will not need your password or email as a Real one would already know this information so there is no need and there's no such thing as an "official discord moderator form" that's just an fancy Excel document Scammers try to make themselves look real
The official discord bot will have a blue box with "Official" next to its name, Not "Bot" or "App"
If the real discord "system" DMs you you will not be able to reply as the reply field is locked, to that DM only interact with the options given
