I was Immortal last season on val. Legit dropping 20-30 kills every single game.
This season I've come on and I legit can't hit the easiest of shots. Only kills I get are crosshair placement kills other than that I couldnt hit someone if they were facing the other way.
Setup feels completely off. Can't get comfortable. Constantly moving my monitor or chair or something just to try and get the feel of my aim back and feel good on my setup. What could this be?

simply stop caring about when ur aim will come back or if u kill or win. peek with the intention of killing no matter the outcome. ur only focus should be timings and enemy's head.
how long was ur break, it could be simply a "coming back" period after a break

You're 100% right bro. Was playing yesterday with a good mindset and my aim wasnt fully back to normal but I was 1000% better than I've been in the past week
