The DS one not going off is kind of a ping thing, I thought. Like the game thinks you're in melee range so it doesn't charge you, but still does the damage. I wasn't ever able to really reliably replicate it, but it usually happened like during a hunters disengage and the game is weird about registering location until you move forward or something.
