well one of the major themes of the game is that the way we judge whether characters' actions are moral or not is how we relate to the people that are being victimised... in the case of the surgeon, we dont care what happens to him because in our mind he's an npc with no important backstory or connections to other characters, whereas what happened to joel was wrong because we spent time with him and felt as though he had justifiable reasons
