you could still get from heroic. It's cumulative with the highest difficulty you do (i.e. doing heroic includes your norm + LFR chance. doing normal includes your normal + LFR chance)

say lfr is 0.1 chance and you only do that that week you have a 0.1 chance but normal is 0.3 and heroic is 0.5. if you do just heroic that week you would have a 0.9 chance because 0.1+0.3+0.5 because it adds the chances of normal and heroic into it

You basically ramp when you can and if you don't have Bloodbath ready. The only exception is, that if you're at 80+ rage and BT is ready, you can choose to BT first if it wont overcap your rage, then Rampage -> BB. Doing this is a small optimisation but it smooths out the overall rotation
