Basically what happens is

• the macro ignores the first line, because Bloodbath is a non-existent spell, just like if you tried to cast Onslaught without it talented
• So it skips that and casts the second line (Rampage) instead
• After Rampage is used, you gain the Reckless Abandon buff and learn the Bloodbath spell, so the macro will now cast it correctly
• After consuming the buff, Bloodbath is unlearned, so it defaults back to Rampage

you can do the same thing with an OF/ONS macro to onlycast the one you have learned at the time (though the tooltips get messed up unless you use learned:##### conditionals)

M+ is pretty much a guess so people do it different ways. You can do regular top gear and just swop out trinkets for burst, do DS, do 5T for 1 minute or so, do all and compare. Do some other 5t sims of varying lengths and compare and contrasts. Will get varying results.
