I always sim patchwerk 5 targets and then the duration of the big cooldown of the class. E.g. 2min DH, 1.5min Warrior. That gives me an idea of the aoe damage in one cd window until i send next cds.

if ur locked into BB - Ramp combo, yes u do ignore it. You basically press it as a filler when you can't BB / BT - Ramp that being said Ashen Juggernaut does place it higher in priority (above BT IIRC)

Definitely weird then. I'm 481 and 2800 and usually just takes about 15-20 mins of spamming to get into one. If I start a 21 or higher I get tons of apps. Not sure what io/ilvl you set.
