You run the sim on the site but the easiest way to get your gear etc is to install the SimulationCraft addon and copy the string it generates
that's how you get stuff like vault options - open the vault, then open the addon and copy the string, then put that into the top gear sim on the site

Yes pretty much everyone needs to use concentration. No the pvp item can't go to 639 (only in pvp), in pve it's 610 max period.
