I find that incredibly hard to believe. I think it's more likely that you're missing the procs than that they aren't happening.

Ignore pain untime as a focus is useless. We use it as a rage dump, and then Brutal Vitality will eventually be self-sustaining with the right number of mobs to damage.
The only time you're actively pressing it is for huge tank hits where padding your HP is useful to avoid spikes even through DR.
And that's rare in anything that's not super high keys or mythic fights.

To be clear, Revenge is your primary rage spender. But by itself, especially during cooldowns, it can't prevent you from capping rage. That's where Ignore Pain comes in.
It will naturally be up a high % of the time, but it's not something you go out of your way to keep up; it just happens naturally as part of the rotation.

you send it on CD to get stacks up, but keep in mind that if you need more time to get back in combat, you can press anvil and it resets the 15 second window
really helpful for some dungeons with a bit more travel time and you dont want to end up at 0 stacks again
