Dungeon scuff aside you're playing incorrectly. Like, physically.
You're hitting your rotation by pressing some combination of 1-2-3 together and hoping the right ability comes out. You're also not SS anywhere close to as much as you can, very often wasting procs. These are an underlying issue to your gameplay and are likely why you're struggling to push into higher keys. I would highly suggest taking some time on a dummy and rebuil that muscle memory into a more deliberate understanding of the spec. I know you know what you're supposed to do, at this point its just teaching your fingers what they need to do
