So first of all, you don't have to be in melee range of the worm at all times. He just spams void bolts basically so you don't have to be there. As far as the corruptors, you basically have two frontals to worry about. One is maddening gaze, which you just watch where they're facing. That's the fear one. The other is when they do Abyssal Reach. That will spawn tentacles that do a small frontal cone. Basically just move ~8 yards in any direction (except where someone else was standing) and you'll avoid it.
