Someone did some stats back in SL; there's a random chance of Tier replacing any slot regardless of you having killed the boss, and once you have killed the boss there's a higher chance of it appearing as loot from that boss

that is super slow, but it's ok, just depends on how much you play and what content you're doing

if you're new to tanking i recommend just running lots of M+ dungeons

also recommend just making your own groups with your own keys

and just running them as high as you feel comfortable

you could probably work your way up to +10 with relative ease at your ilvl if you're new

and use your sparks of ingenuity on items like a weapon and shield first
