It's an extra 'oh shit' that randomly happens when you take semi-normal amounts of damage. It reads like it does more than that, but it doesn't. I'm sure there's specific context that I haven't experienced in a 25 AV, so I'd honestly suggest asking whichever streamer's taking it.

He could have use his defensives more often (shield wall only 1 cast) over a 6min fight, same is true for his Decoration of Flame (trinket)/ravager/Roar
Shield slam cast could be a little bit higher (16 cast per min is reachable, some reach 18) he is only at 50 cast per min where he could be doing closer to 70

I would farm Whetstone if you don't have it already, this one is worth maxing out to 415. You would gain more farming a better chest and catalyst it too.
For gems I have str/hast or hast/vers. It proc vers but it's fine either way.

Also having a good healer can carry you hard. Some classes can also taunt/immun the debuff to give you a break (but it requires communication)
