only time when I recommend defensive stance being on all the time is when you are doing a drastically higher difficulty, than your experience, on a dungeon. Or big/dangerous pulls after cd's
some might not agree, but defensive stance adds a lot of safety to "new" content

So from what I can tell, there are four different stances: Fire, Ice, Wind, and Earth
Each of the stances imparts you with a counter attack that can provide additional buffs.
Each counter attack has the ability to crit for x2 its normal damage.
All of these numbers are based on the 395 ilvl version of the item.
Fire: Counter attack hits for about 10k and puts a dot on the enemies that does an additional 10k over 6 seconds.
Ice: Counter attack hits for about 10k and gives you a buff that heals you for 13k each second for 6 seconds.
Wind: Counter attack hits for about 15k and gives you a buff that increases your haste rating by 1900 for 6 seconds.
Earth: Counter attack hits for about 7k and gives you a buff that increases armor by 3659 for 8 seconds.
The times on buffs may not be perfect, but the other numbers are all accurate for a 395 ilvl trinket.

i think i lean more toward decoration, its basically blood-spattered scale which was insane as well

It's worth crafting a 405 shield. Whether you put armor spikes on it or not is completely up to you, but largely irrelevant either way.

Just watch the timer of dragon strike and use SR a few seconds earlier and you can usually get more in there. If you press it as the cast goes off sometimes it won’t be up for the next one.

is Decoration of Flame up there in terms of a good trinket? What are some good defensive or offensive trinkets?

historically we've only needed D trinkets if we had a specific use for it, otherwise always default to dmg trinkets
frostfire is frost and fire dmg, so every proc will count as fire dmg
if only burning writ was a fire proc dmg
missed opportunity imo to make more enchants more interesting
also disappointing that frozen devo/writ is the better ST option and less for aoe, seems backwards

The difference in secondaries on a weapon aren't the life or death of you, your play is.
Probably just target target, but he might put an icon over them I guess? I can't remember atm
The defensive value of each secondary is also very close, which - coupled with STR being so good - is why ilvl is generally king.

I can keep it around 85% in combat according to Details. I’m mainly pugging in the +10 range and I find I’m running around a lot dealing with bad groups and getting distracted by letting it fall off, or pushing SB twice for both charges

Uptime in Details is not relevant. You want to see your % of blocked hits in logs.

Pushing it twice is not necessarily bad, as long as you're not wasting the duration (meaning as long as you're being hit the entire time it's active)

For 2pc, probably. I personally wouldn't make that sacrifice for 4pc though
