Basically a HDD writes and re-writes addresses tons of times. Doing that creates bits of data everywhere that don't all get removed when that data is overwritten. Your drive head has to move around to read data and if the drive isn't defragged enough, it has to move much more to retrieve data in the proper order.

Not defragmenting slows down performance and as the drive starts to die, increases the chances of data corruption with the read head tripping over garbage data.
