bots repeating the same task even though its finished

i have the problem that when i assign one of my bots with a concrete task, it will complete it and reply in the discord thread, but after replying it replies one more time and says almost the same thing but does not start the actual process i defined in the agents.md
how can i prevent this from happening and also even though i mentioned in the agents.md that the bot should not start any work before i / my discord user preapproves the work, the bot still starts the work. it seems as if the agent does not respect what i wrote in the agents.md file

I'm having the exact same issue with Discord as of a recent update. I have reasoning and verbose both to off. Do you have them off as well?

Yep, both off.

Yeah same here, it doesn't happen all the time either. It started with the Discord enhancements for me.
