I have found that I have to forcefully equip a lot of my items even though they are clearly better than what I was formerly using. The game's logic regarding what it thinks you really want to equip is weird.

For example: I find a new helmet and decide it is better than the helmet I am currently wearing. I try to swap helmets manually, but the old helmet stays in place. I manually unequip the old and try to put the new in its place, but the new doesn't "stick" in the slot. Therefore, I have to put the new helmet in my hotbar and right-click with it in my hand in order to equip it.
