Since no regular attack does any damage you have to slowly whittle him down with bleed and scarlet rot, and then at half health he heals half his hp bar 
Rot does 30% max hp but you really can only get it off once and watch it tick away at the start of the fight, and then bleed does 7.5% of his health whenever it triggers after every time you hit him with swarm of flies, which means you have to get the status to trigger SIXTEEN TIMES

Spikes are like a creeper explosion, closer to the spike, more damage it deals.
Grootslang can enter a berserk phase, and if you are getting comboed the damage it deals to you seem to keep increasing until you break combo.
The debris rain can stack damage very fast, the wave from grootslang hitting the ground can just combo you too...
