Uh, clearance is definitely the reason, the issue is the tunnels in NYC. The NJT multilevel completely fills the clearance envelope — even the similar-looking MBTA bilevels wouldn’t fit.

For me it's mostly a matter of, if it's mostly city/urban running, in cramped places (by OTTD standards), and typically a low-floor type of vehicle, then tram would be better in OTTD terms than a full blown train. Especially if it's only one or a couple of carriages long.
