If the changelog of mod Brothers in Arms (previous gen) is correct, this quest state was originally added by the mod and an attempt to fix stuff. Later when "ng" (Complete Edition) released, CDPR incorporated some stuff from some mods, maybe this too. Anyhow, If it is broken for you, the BiA team might wanna hear about it or often they also have a console command to maybe fix it for you (far as I'm aware, quest states involve something called facts and can be set with console commands).

Multiple fixes for the quest "Little Red": fixed all villagers despawn in front of Geralt's face as soon the quest is over, restored 4 unique one-liners from villagers, and added a new objective to leave the village after the quest is done to properly clean up the phase.

Wait, this sounds way more relevant, because this is from the most recent "ng" version, and sounds like it was a BiA mod bug prior to 3.1:

Version 3.1.0
Fixed the "Little Red" quest not correct closing in certain outcomes of the quest (BIA bug).

Infinite game loading screens are a strong indicator. You travel to a different region for example and game never stops loading. Blurry scenes is also very obvious if it happens, for me it's usually between almost every lines or character switch during "custene-ish" dialoges/conversations. There might be more indicators, but I've only experienced these two kinds, that's not something you can "fail to notice".

i’ve never had this, only thing that happened to me during these last 2 days sometimes, is that i wanted to fast travel somewhere, it started loading and didn’t teleport me and instead kept me at the fast travel post where i was originally

then redoing again it worked

also i sometimes (had it twice now) get a bug where i just can’t save the game at all. only thing that fixes it is fast traveling to a different map, like from velen to vizima for example, then i can save

Never experienced fast travel troubles. Any mods in that regard that you installed? As for "you cannot save right now" message, you get that during moment when the game is already auto-saving or creating a checkpoint or such. Unpause game again and a few seconds later you can save manually again. If your game keeps you from saving for much longer, then I dunno.

i got the fast travel pack mod, and some mod that puts me on the horse after fast traveling. other than that i don’t remember

as for the saving, it’s def some sort of bug or something, because it kept me from saving for like 10-15 mins, prob longer if i kept playing, but i just fix it by fast traveling to another “world” and it fixes itself, only happened 2 times to me so it’s not that bad

The important thing, to start a new game, is only after first time you install a version 3. If already on 3, you can update with your current save and play on.

Might be. Never analyzed it, my understanding was basically any mod can push the game into the bug once you cross a device-specific "limit".
When it happens, I run Script Merger, check bottom right amount of bundle mods and then cut down on them. Usually texture or mesh mods, until fine again.

What causes some games in borderless windowed to not let you move a mouse from one monitor to another without tabbing out? TW3 for instance does that for me but other games like say Overwatch in borderless do let you freely move your mouse across monitors if you're in a menu

i know that some games have options for this, mostly newer ones though i think

usually something called like restrict cursor to game window or something along those lines
