That's just coding limitations. It'd take a lot more work for the NPCs to remember what you've done in your game and if youre in a relationship etc. it just has the same lines based on the action regardless of hearts

If you’re on PC, I will say that milking them is the opposite side of the mouse to petting them and you do have to be near them to milk

If it shows a heart at first cause it made you pet them instead try going closer to them and trying to milk them again if it says they don't have any milk tho idk whatelse could be wrong
