90 focus is a lot to waste on KC. That's 4+ KC's fully just wasted. Your opening SH window is very clean here.
You should only bomb when you have no other available actions during mongoose fury, you could've flanking striked and then MB'd. The order you did it ends up starting another mongoose window when you weren't really prepared for it. You would've had enough focus to finish out that fury window without needing to use the bomb, at which point you'd be at 0, Blue->Red->KC back to full focus
And then at full focus you could begin your fury window again, rather than having to bomb->KC right at the very beginning of it
Some minor GCD issues throughout. These little delays between GCDs add up to lost abilities.
You begin MF there at <50 focus. It's actually preferable to idle for focus instead of starting a fury window earlier if you've completely run out of other actions to do. It's pretty rare that we need to idle for focus ever if focus management is good.
At 54 there you throw the bomb when you could've just MB'd instead (~50 focus). You still do get 2 resets out of it which is good but not all of your reds have so something to be aware of. I wouldn't necessarily just focus dump for it but if you MB instead, could've gotten a proc and just chain into a couple more free MBs and then you've regen'd some focus for another MB and now you can red and get a lot of focus back. (That's how I generally look at that decision anyways, the sim is about the same for using it how you did, vs aiming to use even reds only outside of fury)
Flanking strike has a lot of time off cooldown. That's a lot of wasted focus by the end of the fight if you miss a use of it
Likely as you said, shakiness from being on melee but the flow should be like your opener, MBMBKC, your KC should've been up after those 2 MBs. The 1 MB and then KC again is pretty wonky but you luckily got a KC reset off it but then at 1:48 you throw a blue bomb in your spearhead window
You should pretty much NEVER have to use a bomb in spear head. At worst you use flanking strike if you get unlucky with resets on your KC or not 4p procs, but bombs should never be occuring during SH

Just using this as a highlight of both why making sure your GCDs are tight, and why KC management is important.
The yellow lines are SH begin, and end. You end up with the same number of MB's but you get 1 extra KC inside the window. Your spearhead ended just before that 9th MB which means that extra KC would've extended long enough to catch the 9th MB
Even if your GCD windows were tighter, you still would've caught it
was missed by .1 second
But you can also see how your average focus is higher, so after SH ends, that 9th MB can be followed up by the flanking strike or Kill Command and then more MB's to finish off the fury window by 1:52, then blue -> red -> KC back to full
It's little things like that that add up for SV. Every GCD is very valuable, which means you want to MB as much as you can, but the trick to maximising that is actually maximising KC instead
Key points basically being: KC on cooldown if it won't focus cap you, Flanking strike more often, try not to have GCD downtime. Don't killshot during spearhead. Try to maximise your red bombs when you have them, you want at least 2 KC out of them.

If CA is up you KS with empowerment. If spearhead is up you do not
The spear window is only for KC and MB and if you scuff it, a flanking strike to keep it going as a last resort if you run out of focus due to bad luck with no reset or no procs
