You seem to play a bit of everything. It's very hard to be good a lot of different champions and roles at the same time. Pick 1 role and 1 champion and you make climbing easier. Beyond this, review your games, seriously, you can learn a ton by studying your own games and looking critically at your decisions and even execution. It's a good idea to find and watch full games from high elo 1-tricks on your main champ (in the position you play them ideally) as you will learn lots by osmosis of regularly watching what they do in real, high level games.
