You can sync every single cast of Vend with Flag
Yes, you should hold Flag if you know the fight is 4 mins 30s
But if the fight is like 4 mins 50s, then you would be losing a cast of Flag which is not worth

4 mins is not
But I mean, even at 4:45 you missed out on one cast of Flag
Also, you should be holding Shiv for Vendetta
And only use Vanish after you've used Vend and Shiv on the boss
This is not great.

No, but you should be holding Vanish then
You do not want to Vanish without Shiv
That's why you should be holding Shiv to sync with Vendetta
so you don't delay Vanish too much because Shiv is not available when you Vend

IQD is very meh, especially on longer fights
It's fine on very short fights

okay so the 2nd vendetta cast in the fight was at 2:10 and my shiv was used 1:52. so I held vendetta for 6 seconds not sure why but if i used it perfectly as soon as it came off cd then shiv would've been held for 12 seconds exactly
alright so hold 10-12 seconds for vendetta.
is there any other issues you see? even minor ones?
