One of the dungeons, Temporal Sanctum, lets you take an item with at least 1 exalted (purple) affix on it, and a unique with 1-4 LP, and smoosh them together. It will take 1-4 (based on amount of LP, 1-4) affixes from the exalted item, and put those affixes on the unique, making it legendary. Before it was fully random, but now you can choose 1 mod to guarantee move over, with the others random if you have more than 1 LP

For example, this has 3 LP, so if I took an exalted belt to the dungeon, I could pick one of the 3 mods on the exalted belt, and it would add that mod + 2 of the 3 remaining mods to this belt.

Yep monoliths. Woven echos can also let you customize a lil bit I guess. But yeah if you want say a specific unique bow, you can run the timeline that has increased chance for echo rewards to be unique bows.

Each piece has its own shard. Take set pieces to the woven echo that let's you shatter, think it's the Bridge one? Then shatter pieces, take items that match the set types, so 1h sword if set weapon is a 1h sword. I believe the set shards are prefixes? At least mine all are
