Generally you want to use Shiv on cooldown, and with both rupture/garrote ticking on your main target.

It doesn't really matter if you use Gar/Rup during the Shiv window, especially in multiple target encounters.

If you lust on pull, you just do the single target opener on one target and apply garrote to the second target whenever it comes off cooldown.
