I've posted this issue on the forums but it didn't get much attention (only 1 other person seems to be encountering it). I think there is a bug at the moment where Flagellation sometimes bugs out and skips the first part of the ability and just instantly gives 1 stack of the haste buff? I assumed at first it was being parried, but after further reading I found that it can't be dodged, parried or blocked. Wondering if anyone else here has noticed this?

The reasoning is a little complicated, but the most important of them is the difference in combo points on crit (up to 4 vs 3).
Some other factors to consider are increased poison applications, blindside, and doomblade. This is true for all talent combinations and legendaries.

But you should use common sense
If you have 13 stacks and your Vendetta just comes off cooldown, you might wanna wait for the 15th stack before slamming it
Unless you know you'll miss a cast by waiting.
It's usually safe to hold your very last vendetta/vanish for the unholy proc
Losing casts is never worth though.
