Not always, only until everything is dotted. If the pull lasts long enough, you reapply them. As long as you don’t need to dot more mobs with Garrote and Rupture, you should be using Fan of Knives and Envenom...

Implacable is bugged in Blizzard's Cooldown Manager, there is no way to "fix" it.

Until blizzard fixes it, you just have to either count your envenoms or glance at your buff bar if you're trying to be optimal. Ignoring it when it bugs is <1% loss.
