Preferences currently are wrist/dagger for embellishment depending on what you go for

Darkmoon Sigil Ascension (dagger) + Writhing Banding (wrist) is generally "the best" overall, but Blessed Weapon Grip + Dawnthread can be VERY VERY slightly better in m+

Always craft a dagger for at least an offhand. Put Ascencion or Blessed Weapon Grip on it. Additional Crafts should be Wrist (maybe Belt later in the patch after dropping the delve belt) for your second embellishment, after that whatever low item level slot is fine, necks can be good crafts.
If you have a myth track weapon in your main hand already read the above and pick whatever you prefer

I think you could use vanish before your first envenom after shiv and it dosen't really matter. Its just how i do it. I might be wrong though.

I guess if you have prism you might want to vanish after shiv to get the increased agi for both envenom crits. I don't think it matters much though.

it does, but not that specifically. It shows a general tier list where you can filter by drop source. It doesn't show what trinkets are best for raid and what are best for m+, nor does it show even more specifically what are best for m+ when you don't have access to raid trinkets
