It's currently very buggy. There's multiple things that seem to break it. And a few solutions.

Each of the three cave events can reward a handful of them (Hollow gives two, the most important two for most DPS) and once you have them all, the events will reward a blue quality chest instead.

Something that has worked for me is warmode + a low amount of people. Also waiting 1 - 2 seconds before entering the cave. But a lot of this is probably just vibes rather than anything concrete. The pull boss out and wait method Odon mentions does seem very consistent, too.

Killed the boss with quest up in a raid group and it didn't give me the gem, soon as I wasn't in a raid group and killed him with quest up it worked
