Outside re-reading guides and comparing with other people through vods/logs, we'd need more info to help. you can try wowanalyzer to see if some major stuff is off (likely with the much dps off)

you either use for 2nd, and then every deathmark after, or you use it after the sepsis garrote from your opener, holding the 1:30 sepsis a couple seconds to cover the 2:00 deathmark
and then you would start to worry about when the boss is dying
if it's gonna last a while, you would hold the vanish
if it's shorter you could get away with just holding sepsis for the 4:00dm
and using vanish on cd again
