Check your stealth bar? It might be ambush in place of mut on stealth bar so you think you're muting but actually ambushing, I know this was the problem for a lot of people. Otherwise you can check logs and see what you are pressing after Sepsis. Might be a bit misleading as it won't show ambush vs blindside ambush proc but might be able to sus out what is being pushed
