When playing Exsanguinate, use Puzzle Box just before pressing Exsanguinate. Otherwise always sync Puzzle Box with Deathmark.
Holding Puzzle Box and Exsanguinate for a minute to sync with the 4 minute Deathmark is generally beneficial. Using Puzzle Box on cooldown can be beneficial with very specific fight lengths.

It also works very weird. Basically you get 1 stack per gem of the type per proc. So i.e. you can't get 17 stacks as a result
