I also had this happen today. I could see a potential cause being too much damage/too many sources of damage as it happened in a high level Hunter area with a lot of overlapping AoEs.

Must be under some kind of unique circumstance, since I just had mine trigger normally. Make sure to include as much info as possible to help the devs troubleshoot, like what constellations you have, area/potential enemies/effect that killed you, etc

Could you tell me if you guys had any kind of other death-prevention effect when this happened? Like a star upgrade that gives you a second chance, or Determination Fragment purchase from the souvenir merchant?

No, for me Soul Prison was the only one I had.

Yes, I had souvenir merchant revive. That triggered first. A few seconds later, I died again and my essence triggered as normal
