The Breathe life into doll character

So I managed to play a run with Aurena on Dream Alone and gave the doll 3 souls as per the requirement.... it moved and teleported elsewhere but NO achievement/prompt appeared saying I achieved breathing life into the doll, once I went to the pure white and check the post-game screen, the 3rd character is still locked

was this after you beat the boss or died trying?

This happened to me too I got the 3 souls and the doll vanished after giving it the souls. However, I died to the boss.

I completed beating everything on that run,

but still should've been registered already based on my observation of how others got it
