Every legendary boss gives u the counter next to hp bar and badge number and every abyss trial clear ( no need for no hit ) adds another counter to the Profile badge there are currently 30 bosses so max badge number should be 60

Requirement to trigger the encounter is to unlock all 10 qinghe recipes/food, then try to eat them in front of the stall for that encounter
If you have 2 to be accepted encounter no matter how many foods you ate it won't trigger since you can only have max 2 to be accepted encounter

NMW's base is the most efficient place for mushrooms. But this area in a village outside of Kaifeng has a cluster of about 16 mushrooms iirc.

The best way though, for me, to farm poison powder is to do the repeatable outpost farm at the Eastern side of Kaifeng, is the Mistveil City area. Repeatedly blow up the barrel and look the 4 poison mobs, use lightness skill and drop back down to repeat after they respawn.

If it's indeed a bug, relogging should solve the issue. Otherwise another thing he/she can try is to complete the Mistveil Divinecraft Dungeon nearby and see if that unlocks it.

I usually leave bosses till the end, so that may be a hidden pre-requisite, though I doubt it.

im trying to do heartseeker trial, and i keep getting locked into just my main weapon, cant use secondary, inner ways are inactive, mystic skills disappear too
