No treasure chest from spire

I was killed and returned to a spire, defeated it but no treasure box spawned

Hey Belle. Did you make sure to eliminate the followers that were around Spire?

This happens to me to sometimes and even if I defeat both the Spire and the followers I get no chest, that's why I just left all the Spires I've found untouched

I have this happen to me, 5 spires nothing to show for it and they don’t come back
