Your running driver version 595.97 (latest correct)?
We are tracking an issue with only that driver on the 40xx cards.

Can you try 591.85 please - this seems to stop the crashes caused by GPU.

Secondly, check your CPU settings, some motherboards have a boost that goes a little to far when a game trys to use 100% of the cpu, we see this exception crash.
